Freedmen's Medicine brings primary care, behavioral health, substance use treatment, and help with everyday needs to homes and community settings across the District of Columbia. It provides one integrated team that comes to the person, closing the gap between medical and behavioral health care. It is the implementation partner for Promoting Integrated Whole Person Care – DC (PIWC-DC), a five-year SAMHSA-funded initiative through the DC Department of Behavioral Health aiming to bring integrated care to 10,000 District residents by 2029.
