The Cognitive Connection began in 1995 when Clay, Wilson & Associates set out to help improve the lives of adolescents and adults living in the Unifour area. It provides integrated health and wellness services including primary care, behavioral health, care management, and adolescent services, and has been awarded contracts to develop and provide cognitive behavioral intervention, employment training, substance abuse treatment, education, and case management programs throughout North Carolina.
